Referral Pathway for Professionals

At Home-Start Knowsley, we value the vital role professionals play in identifying families who may benefit from early, compassionate support. This referral pathway is designed to make it easy for you to connect families with our services and ensure they receive the help they need.

Who Can Refer?

  • Health Visitors
  • Midwives
  • Social Workers
  • Early Help Teams
  • GP’s
  • Specialist Services
  • Schools and Nurseries
  • Other voluntary and statutory services
  • Parents can self-refer

Before Making a Referral

  • The family lives in Knowsley and has at least one child under 11.
  • You have discussed Home-Start support with the family.
  • You have obtained the family’s consent to refer.

How to Refer

  • Complete one of our Referral Forms – Please use Flourishing Futures Referral for Perinatal Families 
  • What happens next? Once we receive your referral, one of our Coordinators will assess it and contact you with the next steps.

Support We Offer

  • 1-1 Individualised Support – Emotional and practical support in the family’s home and support to access other services.
  • Flourishing Futures– Support during the first 1001 days (Pregnancy to 2 years)
    • Parent & Baby Yoga – 5 weekly sessions of Mother and Baby yoga for pre-crawling babies.
    • Baby Massage – 1-1 when required.
    • Baby Sensory – A sensory class for Parent/carers with babies 0-12 months old where babies can explore different textures and sounds whilst bonding with their caregivers.
    • Bump and Baby – Supporting Knowsley Parents from pregnancy – 2 years throughout Knowsley – See timetable for times and venues.
    • Love, Grow & Bond – 7 week course focusing on:
      • Bonding and Attachment
      • Baby’s Brain Development
      • Healthy Relationships
      • Safety in the Home
      • Taster sessions of Baby Massage, Baby Sensory and Parent & Baby Yoga
    • Trauma Response Support – 1-1 sessions specifically focusing on past trauma delivered by 12 Million Minds.
    • Flourishing Minds – 5 week course for parent and baby focusing on emotional wellbeing, meditation, and sound therapy.
    • Singing Mamas – 10 week singing for wellbeing sessions for mum and baby delivered by Singing Mamas.
  • Parent and Toddler Group every Thursday morning during term time
  • Holiday Allsorts – Family fun activities/days out during school holidays.
  • Hardship Assistance and Fuel Vouchers – We currently have Household support funding to assist with food and essentials.
  • Counselling Service – In-house counselling service provided by BACP Registered Counsellor.
  • What Now? Peer support group for adults with late neurodiverse diagnosis/on the pathway.
  • Time for Me Mindfulness and Wellbeing sessions for adults. example of sessions, wreath making, aromatherapy, baking, cooking, introduction to hypnotherapy.
